If i format a machine with Modul8 authorized on it, will my SN still work when I have re-installed the OS ? Or should I uninstall Modul8 and ask for a new SN when I am done re-installing the OS ?

Modul8 registers your machine ID, so it will be no problelm.

Yes. As long as your motherboard is not replaced, it won't count as an extra machine (even if you re-register the same machine).
