We stated 64 universes to be conservative, but I'm pretty sure the MiniMad can handle more than that and 96 should be ok.

Did you make any change to the Midi devices in macOS through the "audio MIDI Setup" app?

Additionally we have been unable to get our Blackmagic Design UltraStudio 4K Mini to show up in madmapper on the last few versions on any of our machines.

If you want to access the MiniMad through the internet, you either have to configure your internet-router to forward the right ports (being several Samba ports and OSC port 1111) to the MiniMad, or better install a VPN service on the network.

MiniMad was released way before the cues actually arrive in MadMapper. MadMapper will export the surfaces in their current state at the time of the export. If you want to shift between different surfaces states, I advice you record MadMapper's output using a Syphon/Spout/NDI video-recording software...

I've tested it and you're right, the folder is created even though all related-features are disabled.
